Please select any that apply to your pet. Honesty is important and helps us get you booked with the groomer who will best fit your pet's needs. No selection is needed if none apply.

Does your pet have medical needs, or do they need any special accomidations? Our goal is to create a positive grooming experience, so let us know how we can help. If you have a specific groomer request, please let us know here.
What setting is your pet used to being groomed in? Knowing their history helps us decide where they will best fit into the schedule. If your dog is used to mobile, house call etc. we may try to book them at a quieter time in the day. If you pet is used to a corporate salon, we will know they are used to high traffic settings.
Why are you searching for a new salon? This helps us determine if we will be a good fit for you and your pet.
What Service Does Your Pet Need? Nails and de-shedding (if needed) are included with every appointment. We will not add a haircut last minute.

Most of our clients have a standing appointment every 8 weeks or less. If you are happy with the first service, is this something you'd be open to doing? Saying no doesn't mean we will not accept you as a client- but we may prioritize clients who want to stay on a schedule.

Upload RABIES certificate here. NO TAGS! NO PHOTOS TELLING US YOU WILL GET RABIES LATER. This system is how we stay organized, and we are unable to proceed with booking without it. We will not take expired vaccines. If your puppy is under 6 months, please upload most recent shots. If you have a PDF, you will need to take a screenshot and upload it in photo format.

I will fully read and sign the service waiver when my groomer sends it to me after i book my appointment. Our policies can be found on our website as well. I understand the deposit to book is non refundable and non transferable.
